ADHS Immunization Record Requests
A valid email address is required to submit a request.
All immunization record requests must be accompanied by documents that identify the person requesting the immunization record. Examples of acceptable forms of identification are:
- a state-issued photo driver's license
- a state-issued photo identification card
- a passport booklet or passport card
Records for individuals over the age of 18 must be requested by the individual themselves or their Healthcare Provider.
Records for minors under 18 years of age must be requested by the guardian of the minor or their Healthcare Provider, please add the minor's information in the Personal Info section and your information in the Requester Info section.
